The Health Services Director is responsible for all aspects of the Assisted Living and Memory Care programs offered, including staffing and the coordination and provision of care for residents who are receiving Assisted Living services. The HSD also adheres to MG customer service philosophy by providing excellent customer service to residents, their families, and team members. Must be a licensed RN. Compensation for this position is competitive, DOE.

Essential Duties and Responsibilities

Provide a high level of customer service to residents and team members by achieving the standards outlines in the MG CARES program.

  • Overall 24/7 accountability for clinical care coordination for residents in AL & Garden House, ensuring that all programs and services are delivered according to the philosophy and standards of Merrill Gardens.

Resident Services:

  • Develops and implements the highest standards for Assisted Living services provided to residents.
  • Works directly with the residents and evaluate needs and services.
  • Conducts and/or assists with electronic evaluation of incoming residents, and periodic re-evaluation of residents, to ensure services are documented and charged for according to Merrill Gardens' Policies.
  • Participants in the development and implementation of resident services and care plans for all residents.
  • Makes rounds to verify residents are provided services according to their identified service needs within Merrill Gardens' policies and state requirements. Assists with resident care needs as required.
  • Observes residents' symptoms, reactions to medications and/or treatments, changes in condition and unexpressed needs. Takes appropriate action and notifies physician and family/surrogate of decisions made when significant changes in resident's observed condition occur.
  • Oversees preparation, administration, and documentation of medications and treatments as ordered by the residents' physician and in accordance with federal requirements, state regulations, and facility policies and procedures. Orders medications from the pharmacy in accordance with prescribers' orders.
  • Conducts monthly drug re-cap according to facility policy and procedure, counts controlled drugs, and disposes of discontinued medications according to state regulations.

Supervisory Responsibilities:

  • Responsible for hiring, training, managing, evaluating, disciplining, and terminating all Caregivers and Medication Assistants
  • Determines adequate staffing levels and prepares staffing schedules using approved tools.
  • Replaces absent team members as required per resident care and facility needs.
  • Ensures proper training and certification of all nurses and medication assistants as required by Merrill Gardens and state and local guidelines.
  • Mentors team members using electronic care system.
  • Supervisors nurses and medication assistants to evaluate performance on an ongoing basis to ensure residents receive appropriate care.
  • Provides staff in-service training, as indicated by the facility and resident needs, to ensure team members have received training necessary to accomplish their assignments and provide accurate resident care.

Administration:

  • Adheres to budgetary guidelines established by Merrill Gardens.
  • Provides General Manager with weekly status reports on AL residents.
  • Maintains all resident records and department documentation.
  • Audits AL services for regulatory compliance.
  • Reviews and verifies all Nurses and Medication Assistants are using the time clock appropriately and taking breaks as indicated by policy and regulations.
  • Responds quickly to all emergency situations, including but not limited to: Assisting in fore control and resident evacuation procedures during fire/disaster; Ensuring care personnel know action to be taken during fire/disaster; Perform CPR as necessary.
  • Meets continuing education requirements when necessary for state licensing/registration.
  • provides additional management reports as required.
  • Ensures the AL work area, which may include offices, medication rooms, etc.., are clean and orderly.

Job Type: Full-time

Pay: From $72,000.00 per year
