**This position will be a combination of in-person and tele-health work in Winchester, Frederick County, Shenandoah County and Warren County**

Connected Communities Inc. is seeking a full or part-time licensed or licensed-eligible Therapist in the Winchester, VA area to provide outpatient counseling to children, adolescents, and adults with identified mental health diagnoses. Therapists must have strong interpersonal skills and ability to build trusting therapeutic relationships with clients. Candidates will possess strong written and verbal skills and clinical knowledge regarding diagnoses and effective, evidenced-based treatment. Candidates will demonstrate an ability to work effectively with community providers and uphold high standards of best practice.

Therapists will be provided with licensure supervision, credentialing and training to meet yearly CEU requirements. This position requires at minimum 1 night per week of evening appointments but also provides opportunity for telecommuting hours as well.

The Outpatient Therapist provides outpatient therapy and related clinical services to children, adolescents and adults.

DUTIES AND RESPONSIBILITIES:

  • Implements and maintains Connected Communities’ philosophy, principles and procedures in all facets of performance
  • Manages a caseload of clients, providing services on a weekly and/or bi-weekly basis and utilizing evidenced-based treatment strategies.
  • Prepares written diagnostic assessments from a Biopsychosocial perspective and focuses on clients strengths and needs including family, educational, social, medical and personal functioning and utilizing various assessment instruments.

Completes IACCT Assessments for youth being evaluated for residential treatment including coordinating with parent, community providers and physician to determine recommendation for appropriate level of care and participation in recommendation meetings as necessary.

  • Prepares written treatment plan, that includes input from clients, community service providers, and other team members, clearly identified areas of needs and strengths, evidenced-based strategies, and measurable objectives in a timely manner that meet audited tested requirements
  • Assure continuous reimbursable sessions for caseload of clients
  • Maintains calendar with appointment times and client initials for billing purposes
  • Answers emails within 24 hours
  • Returns phone calls, text messages and other communication within 24 hours
  • Completes progress notes within 24 hours of sessions and turns in progress notes at the end of each week
  • Updates Treatment Plans on a minimum of a quarterly basis
  • Provides Care Coordination with other treatment providers on a monthly basis
  • Attends Individual Clinical Supervision on at least a bi-weekly basis
  • Attends Group Clinical Supervision on a monthly basis
  • Submits timesheets by the 1st and 15th of the month with a 3-day grace period for holidays, weekends, vacation, etc.
  • Complete serious incident reports within 24 hours of incident

Requirements:

  • Licensed or licensed-eligible in Virginia (LCSW or LPC) or Resident in Counseling registered with the Board of Counseling in Virginia
  • Master’s degree in human service related field (e.g.: Social Work, Psychology, Counseling)
  • Planning, time management and strong organizational skills
  • Ability to maintain confidential information within HIPAA guidelines and organizational policies
  • Strong Oral and Written Communication Skills
  • Professionalism
  • Demonstrates integrity and empathy
  • Strong ability to build relationships with others
  • Basic knowledge of the Therapeutic Process and Clinical Counseling Skills
