Summary: This is a direct service position requiring general and specific knowledge of mental health, community programming and entitlement programs. Primary responsibilities are to provide and/or coordinate assessment, planning, linkage, monitory and advocating for and with enrolled adults.

Responsibilities:

• Provide and/or coordinate the following services for and with assigned individuals: assessment, planning, linking, monitoring, advocacy and care both internal to Way Station's program and through external social service agencies.
• Be available to address client concerns as they arise on any given day (i.e. resource management, appointments, transportation, housing, skills teaching, entitlements coordination, member debt, money management, etc.).
• Accompany clients to appointments as needed.
• Maintain current, accurate and complete documentation related to all services and functions (i.e. entitlements, enrollment requirements and linkages to internal and external services).
• Obtain and maintain entitlement linkage for clients as assigned.
• Connect clients with relevant external agencies as needed.
• Collect income verification copies of social security card, birth certificate, etc.
• Provide initial transportation for clients and assist with coordinating transportation options/exploring community resources.
• Assist clients and staff with transition process of new enrollees to rehabilitation services.
• Complete enrollment checklist, obtain client signature on contracts and agreements.

Requirements:

• Possession of a bachelor's degree in related field or associate's degree and relevant work experience that uniquely qualifies the individual for the job.
• 2 years of relevant work, student or volunteer experience.
• Must have valid full driver's license and safe driving record. (License cannot be provisional).
• Possession or use of a four passenger car to be used to transport clients.
• Must be covered under an active auto insurance policy and a valid copy of auto insurance is required prior to hire.
