Job Summary:
The Medical Practice RN Care Coordinator is responsible for a variety of case management duties in the outpatient setting including but not limited to managing chronic care patients, assisting in meeting ACO initiatives, and being a resource for the primary care and behavioral health teams. She/he offers support to the Medical Practice Clinical Coordinator. This nurse is responsible for utilizing the Nursing Process to ensure that quality care is provided to adult and pediatric patients.

Essential Qualifications: Graduate from an accredited School of Nursing. Current NH Nursing License (RN). BLS certification is required. A minimum of three to five years of experience in a clinical or community health setting with care coordination, case management, or home health experience is preferred. Must have excellent communication, organizational, and problem-solving skills and possess personal qualities such as kindness, understanding, respect, good judgment, and integrity, a sense of humor, enthusiasm, and ability to relate effectively to patients, families, and co-workers. Able to speak a relevant secondary language is preferred.
