Registered Nurse / Case Manager & Training Manager Job Description

Guardian In Home Health & Security, LLC is looking to add a part-time Registered Nurse who is looking for a rewarding career to join our team! RN's will manage client's care as well as coach & train caregivers. In this position you will have the freedom to create your own schedule!

Registered Nurse / Case Manager & Training Manager Client locations vary, but areas include:

• Watertown, Glencoe, Hutchinson, Buffalo Lake, Gaylord

RN / Case Manager & Training Manager Responsibilities:

• Management of client's medical needs at home
• Complete an assessment of client, create a care plan by obtaining Doctor's orders, and advocate for client
• May include attending Doctor's appointments with client
• Maintain and complete timely records, written and electronic
• Train, Supervise and Follow-up on care-giving staff
• Work approximately 10-15 hours per week (or month)
• Reimbursed Mileage & Paid drive time

Registered Nurse / Case Manager & Training Manager Requirements/Qualifications:

• Licensed as a RN and eligible to practice in Minnesota
• Reliable transportation
• Valid MN driver's license (or license that matches your current permanent residence)
• Current and adequate auto insurance
• At least 2 years experience managing patient care
• At least 2 years training staff
