ApolloMed is looking for a LVN Supervisor - Hospice tp work at our Rosemead, CA location at 3505 Hart Avenue. You will be required to travel to patient homes, but you will be reimbursed at the IRS mileage rate. This position's schedule is typically M-F from 9am to 5pm but you will also need availability to work outside of those hours and during weekends if there is patient need.

While you will not directly supervise anyone when you start, you will have the ability to obtain supervisory responsibilities as we grow. This position is a great opportunity for a newer LVN to grow their responsibility!

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Collaborates with Director of Nursing to develop Plan of Care for patients, implement IDT plans and attend IDT meetings.
• Participates in all nursing and clinical services, including direct nursing care, patient care coordination, and QA of Plan of Care and nursing notes.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Coordinates patient care with different vendors such as nursing homes and pharmacies.

REQUIRED:

• Active and unrestricted LVN license in California
• CPR certification
• One (1) year of clinical experience
• Ability to drive to client sites in California
• Flexible schedule and weekend availability

• Ability and desire to work in a fast-paced environment
• Understands philosophy of hospice concept and needs of the terminally ill
