Overview:
The RN Oncology Coordinator will act as the liaison between the patient and the members of a multi-disciplinary cancer treatment team to ensure the patient will receive seamless care provided throughout the Mercy Health System. He / she will give support; provide guidance and treatment coordination for our oncology patients, their families and / or caregivers. The coordinator will also act as an advocate and educator, providing explanations and insights, to ease the stress and burden of complex cancer treatment plans starting at diagnosis through survivorship. The coordinator helps remove barriers, improve timeliness to care and connect patients to support services. The oncology coordinator becomes the main point of contact within oncology services. Performs other duties as assigned.
Responsibilities:
  • The role of the Coordinator is to assess newly diagnosed patients to get a complete picture of the patient’s comprehension and understanding of their diagnosis and how to best guide the patients through their treatment and beyond.
  • The Coordinator coordinates and participates in MD appointments, as well as scheduled tests, procedures, and imaging during the initial diagnosis and treatment planning phases.
  • Coordinates all facets of comprehensive care including but not limited to: - Oncologists (medical, radiation & surgical) - Social workers / case managers and the psychosocial services within our cancer program and out in our community. - Patient financial counselor to help with insurance coverage and potential financial assistance that may be available. - Educational resources that are specific to each individual case - Ancillary services: PT/OT, Lymphedema clinic, Clinical trials, Genetic testing, Nutrition, Pastoral care, Support groups, Speech, Interpreter services, Complementary medicine, Palliative care and Hospice etc……..
  • Will educate, coordinate, support and develop a trusting relationship to help navigate our patients through Mercy Health System Cancer Program and Services.
  • Will be knowledgeable about NCCN cancer treatment guidelines, FDA approved treatments, studies available within and outside our facilities.
  • Will possess strong written and verbal communication skills.
  • Must be detail oriented and organized.
  • Must be proficient in Computer record keeping: proper EMR documentation on all patients and in keeping accurate and up to date records on patient encounters by phone, in person or via e-mail.
  • Must implement a computer tracking system for data collection and benchmarking performance to identify patient successors and improvement areas.
  • The RN Oncology Coordinator will perform the duties of the Oncology RN/Staff role should needs arise.
Education and Experience:
Graduate of an accredited nursing program. BSN or upon hire BSN completion agreement for enrollment in an accredited BSN Completion Program May require specific years of applicable experience to qualify (per department guidelines). If hired prior to 1-1-14 in Winnebago County and prior to 9-1-14 in Rock, Walworth and McHenry Counties an ADN only is required.
Certification/Licensure:
Licensed as an RN in the state of practice
OCN certified – required
Special Physicial Demands:
While performing the duties of this job, the employee is frequently required to stand, walk, sit, reach, handle, finger, feel, talk, and hear. The employee is occasionally required to stoop. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust and focus. The employee must exert 25 to 50 lbs of force occasionally or 10 to 25 lbs of force frequently and be able to lift 10 to 25 lbs occasionally.
