HOURLY RATE STARTING AT: $27.00 per hour for LPN's and $35.00 per hour for RN's

Position: RN or LPN

Shift: PM shift

Northern Lights Services specializes in the following services:

Skilled nursing and post-hospital care
Short-term rehabilitation (in-patient & out-patient physical, occupational, and speech therapy)
Long-term care
End-of-life care

Position Summary:

• To coordinate timely and accurate nursing assessments, interventions, and evaluations with supporting documentation in the resident’s record to assure that desired resident outcomes and standards of resident care are provided.
• Assist in planning, directing and providing resident care as a front line supervisor/leader. This will include but not limited to medication administration, data collection, assessment, provision of treatments, risk assessments and implementation, supervision of staff and care plan development.
• Recognize signs and symptoms of change of condition – respond as indicated by resident need and follow through with appropriate contacts to include clinical manager, family and MD.
• Lead and supervise nursing assistants and caregivers. Establish communication channels, organize and assign duties and assignments, provide problem solving, leadership skills and conflict management as needed.
• To coordinate and schedule follow-up assessments/interventions with other staff.
• Attendance of required staff meetings, daily report and continuing education sessions in order to be informed of current processes and plan to provide quality resident services.
• Must be able to work positively with team members and support all aspects of team approach.
• Orientation and training of new staff as needed.
• Abide by all facility rules and regulations.

Qualifications/Education:

Must possess and maintain a current Wisconsin RN or LPN license. Prefer management and supervisory experience in Long-term Care and comprehensive knowledge of Long-term Care. Must be able to demonstrate competence/proficiency with computers and certain medical equipment. Must be CPR certified.
