Hospice of Lenawee is hiring for a:

  • RN - Part-time - Day Shift - Hospice Home*

(Day Shift is a 12-hour shift: 7a - 7:30p)
This part-time position will be scheduled for 1-2 shifts per week with an every 3rd weekend rotation and a holiday rotation.

Essential Job Responsibilities of an RN:

  • Provides skilled nursing care based on sound organizational, analytical and problem-solving techniques, and accepted standards of care.
  • Develops a comprehensive care plan that addresses physical, emotional, cultural, and spiritual needs of the patient and family based on input from the patient, family, and interdisciplinary team.
  • Assesses patient and family response to related plan of care interventions.
  • Aggressively advocates for pain and symptom management.
  • Teaches, supervises, and counsels the patient and family regarding care issues.
  • Directs and supervises LPN and Hospice Aide.
  • Assists with patient personal care, laundry, dietary, and housekeeping services as needed.
  • Fulfills responsibilities of Hospice Aide as needed.
  • Maintains close communication with attending physician and IDT members involved in the patient’s care.
  • Administers and documents medications and treatments as ordered by the Physician and according to HOL policies and procedures in an accurate and timely manner.
  • Exhibits willingness and flexibility to work different and/or extended shifts when needed.
  • Identifies and responds appropriately to emergency situations.
  • Reports all accidents and incidents to the Residence Manger or supervisor on call and completes required documentation.
  • Assumes responsibility for compliance with federal, state and local regulations within the scope of practice.
  • Assists with orientation of new staff members as requested.
  • Performs admission of patient to hospice care when needed.
  • Attends shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patient’s care.
  • Meets safety needs of patients and uses equipment safely and appropriately.
  • Documents and records care provided according to agency standards.
  • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication, and performance of duties.
  • Be knowledgeable of and follow policies and procedures set by Hospice of Lenawee.
  • Holds all patient information in strictest confidence and discloses information and data only to persons authorized by Hospice of Lenawee.
  • Performs other duties and responsibilities as determined appropriate by supervisory staff and/or management.
  • Demonstrates willingness and flexibility to work different and/or extended shifts when needed.
  • Presents appropriate professional appearance in accordance with Hospice of Lenawee policies.
  • Participates in weekend and Holiday rotation.
  • Hospice of Lenawee direct care staff may have exposure to infectious diseases. Must comply with standard precautions to prevent the spread of infectious disease.
  • Attends staff meetings and completes all mandatory training.
  • Complies with attendance policy and is physically at assigned location to fulfill job responsibilities and functions.

Qualifications of an RN:
Required Knowledge, Skills, and Abilities of an RN:

  • Demonstrates ability to make sound clinical judgements and act upon them in a timely manner.
  • Demonstrates ability to manage time effectively and to prioritize immediate needs of patients and families.
  • Proficient with technical abilities such as IV administration, equipment function and maintenance.
  • Demonstrates ability to plan, assess, implement, and evaluate nursing care for patients and families.
  • Demonstrates ability to function autonomously in the Residence setting within the policies and procedures of HOL.
  • Demonstrates excellent customer service, both internally and externally.
  • Possesses strong communication skills and the ability to interact positively and professionally with patients, families, and other team members.
  • Demonstrates ability to function as a team member, collaborate with coworkers on different shifts, positions, and departments to promote teamwork and inclusion.
  • Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
  • Is self-aware and asks for help when the need arises.
  • Demonstrates ability to read, write, and follow direction and instruction.
  • Has the physical ability necessary for extensive standing, lifting, and pushing.
  • Demonstrates ability to prioritize and manage time effectively.

Education/Credentials of an RN:

  • Graduate of approved School of Nursing.
  • Current Michigan Registered Nurse License.
  • BSN degree, preferred.
  • CPR certification, required.
  • Food handler card, required.

Experience of an RN:

  • Two years clinical nursing experience in hospice, home health, or oncology nursing, preferred.
  • Experience with dying patients and their families, hospice experience preferred.
  • Experience in health care, required.
