Job Summary:
CareLinx by Sharecare and a large health plan are partnering to provide members with in-home care benefits and are looking for a caregiver in the state of California. As a nurse you will be supporting enhance care management programs by helping with HRS assessments, Care Planning, Medication Adherence, and basic medical nursing functions. You’ll be travelling to multiple client’s homes & provide the support needed every day. We are looking for experienced and interested nurses who are ready to help out as needed.

Essential Job Functions:

• The primary duties of this registered nurse will include:
• In-home/Onsite HRS (Hazard Ranking System) Assessment
• Assist in transition care planning
• Medication adherence
• Basic medical nursing functions such as vital signs, wound care, vaccination, ADL housing assessment
• Traveling to multiple clients’ homes or places of service every shift.
• Chronic care management preferred
• Perform initial evaluation and periodic re-evaluation of patient's conditions and recommend revisions of the patient's plan of care as needed.
• Prepare clinical notes in accordance with professional and agency specific standards.

Qualifications & Job Requirements:

• Previous medical nursing experience or anything related
• Has an active In-State/Compact License
• Reliable & professional
• Personal transportation for quick mobilization from one client’s home to another, additional transportation allowance/mileages included
• Computer, internet access and email capabilities from home or other off-site location
