We are seeking a full-time (1.0 FTE) RN, Triage to support our Ashland Clinic Team!

The Triage RN will provide triage and health advice by phone to patients, utilizing evidence based triage protocols, as well as processing prescription refill requests received from pharmacies and patients.

The Triage RN duties include but are not limited to:

• Performs nursing assessment and triage. Follows clinical guidelines, protocols and policies related to direct patient care.
• Complete refill requests for non-controlled substances per NLCC protocol and guidelines
• Complete refill requests for controlled substances under direction and approval from prescribing provider
• Promote timely access to appropriate care to reduce emergency room utilization and hospital readmissions
• Connect with patients for ED and hospitalization follow-up
• Provide medication reconciliation
• Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care cost
• Facilitate patient access to appropriate medical and specialty providers
• Educate patient and family/caregiver(s) about relevant community resources
• Attend Care Coordinator training courses/webinars and meetings - Provide feedback for the improvement of the Care Coordination Program
• Participates in quality improvement efforts including outreach, utilizing dashboards, tracking tools and providing quality reporting data monthly
• Document in the Electronic Health Record

Qualifications and Education Requirements

• Associates or Bachelor’s degree in Nursing required
• Two+ years related experience and/or training
• Excellent communication, education, and counseling skills
• Local knowledge about connections to community health care and social welfare resources is desirable
• Experience with electronic health records and triaging patient preferred

Required Credentials, Certification, Licensure

• Current license as a Registered Nurse in the State of WI required
