Job Summary:
Primary function is to schedule patient/client services and coordinate with staff.
Requirements:Job Qualifications:
Education: High School graduate.
Experience: One year experience as a home health scheduler preferred.
Skills: Exceptional communication skills. Experience with electronic medical records, HCHB preferred. Ability to multitask in a fast-paced environment.
Transportation: Reliable transportation.
Environment and Working Conditions:
Works in routine office environment. Noise level may be moderately high, ability to work a flexible schedule and extended hours. Some exposure to inclement weather.
Physical and Mental Effort:
- Prolonged sitting and some standing required.
- Occasional lift, pull, carry and push items weighting up to fifty pounds.
- Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions.
- Requires hand-eye coordination and manual dexterity.
- Required problem solving skills.
Essential Function:
- Evaluation
- Schedules and appropriately documents patient/client schedules in a timely manner.
- Participates in coordinating care with management and patient/client interdisciplinary team.
- Provides effective communication to patients/clients, staff members, other health care professionals and referral sources.
- Processes reports timely to ensure compliance.
- Responds to Agency and patient/client needs in a professional and creative manner.
- Promotes the agency philosophy and mission by presenting a positive image to patients, families, physicians and community agencies.
- Demonstrates commitment, professional growth and competency.
- Provides effective communication to patient / family, team members and other health care professionals and maintains confidentiality.
- Performs other duties as assigned.
