Introduction


A case manager at the City of Middleton works in a team-oriented environment and is provided with regular support from other case managers within Dane County focal point system. The facility is committed to acting as a resource for community partners as well as internal stakeholders. Regular training and networking is also provided through our Dane County focal point resources. The program provides advocacy for clients by using a variety of services available across Dane County, as well as a county mental health resource team. While the position is in person and is 20 hours per week a flexible work schedule can also be provided.

Position Summary


The Senior Services Case Manager, under intermittent supervision of the Senior Services Director, provides front line services and contact with older adults. The Middleton Senior Center is both state and Nationally Accredited, setting a high standard for services provided for those 55 and over. These services are on a one-to-one basis, to determine the needs, concerns, and interests of older adults. This position helps older adults overcome financial, personal, health, and/or family problems, and provides personal and community support for them, and their caregiver network. Case managers work under the support of the Dane County ADRC and the City of Middleton through a shared partnership supporting older adults so they can remain in their own homes longer.

Salary Information


$23.28 - $26.77 Hourly, No Benefits

Job Details

  • Assess the needs of older adults who are referred to the Middleton Senior Center for programs such as home delivered meals, adult day care, the Community Options Program, telephone reassurance, friendly visiting or other long term care service.
  • Complete any necessary written forms or applications as required to acquire the necessary services and programs and conduct appropriate follow-ups.
  • Work with families, friends, and neighbors of older adults during the assessment process to determine the types of services that could support the formal service with informal supports and care giving.
  • Develop written service plans for those persons assessed and follow up with the person or their family to ensure the service plan is being carried out.
  • Determine skills, talents and interests older adults might share with the community.
  • Serve as an advocate for older adults in dealing with social service agencies and government bodies.
  • Record on the assessment forms any gaps or problems in delivery of services or any other information that may be important to Dane County aging agencies in planning and evaluating services.
  • Maintain records of contacts made and actions taken in working with older adults and ensuring confidentiality of those records.
  • Maintain monthly records and reports necessary for Dane County and the City of Middleton.
  • Maintain a regular schedule of office hours for public contact.
  • Attend monthly training sessions provided by Dane County Aging Program and other training when available and affordable.
  • Serve as a resource person to all Senior Center programs where case management services may be appropriate for referral. Case Manager will also serve as a source of information on aging services available to the community.
  • Manage inventory and distribution of Durable Medical Equipment Loan Closet.

Qualifications


Knowledge

  • Knowledge of state and federal benefits including housing programs.

Skills

  • Capacity to facilitate interaction and cooperation between older adults and community resources and services.

Abilities

  • Ability to communicate with older adults, their families, and other agency staff.
  • Ability to travel and complete home assignments.
  • Initiative and ability to plan and work with little supervision.


Education

  • Bachelor's degree in Social Work with a Certified Social Work Certificate preferred.

Training

  • Social Work Training Certificate required.

Experience

  • Three years of practical experience working with older adults or education equivalent.
  • Experience in completing a needs assessment, preferably with older adults.
