UVS is currently seeking a full-time, benefited Service Coordinators. Qualified candidate will possess the skills to enthusiastically provide person-centered planning services to individuals with developmental disabilities. The Service Coordinator is responsible for development and monitoring of person-centered plans that demonstrate a person’s goals and dreams, collaborative support strategies, and placing value on naturally occurring relationships. Coordination of individualized, high quality supports for individuals on assigned caseload. Assuring the overall health and safety of individuals served is balanced with supporting the individual’s self-determined life of informed choice, life-long learning, empowerment, and the opportunity for meaningful contribution within the community of their choice.
The position requires the ability to work independently, communicate effectively with various community partners, prioritize work and develop systems that satisfactorily support all individuals.
The preferred requirements include a Bachelor’s Degree in Human Services or a related field and a minimum of two years of experience working with individuals with developmental disabilities and their families.

Upper Valley Services offers flexible schedules, hybrid schedules, great benefits and a respectful and collaborative work environment. The starting salary for this position is $45,000 a year. A $500 hiring bonus will be offered to the selected candidate.
