JOB TITLE: Service Coordinator (LPN), Herkimer Gardens

REPORTS TO: Program Supervisor

DEPARTMENT: Mixed-Use Affordable Housing

HOURS: 35 hours per week

QUALIFICATIONS:

Licensed Practical Nurse, Experience working with people with disabilities and aging population a plus. Fingerprinting, criminal record check, approval from NYS Office of Mental Health

JOB FUNCTION: Provide assistance to individuals age 55+, with chronic health conditions who need assistance with activities of daily living, to manage their medical conditions, maintain independent living and promote community inclusion. Provide support and education, coordination of care and interface with medical providers and community resources to assist individuals served.

Job Type: Full-time

Pay: $70,000.00 per year
