Position Purpose

Responsible for the development and delivery of services and activities for seniors at Barrio Viejo housing complex. Maintains all HUD Standards for Service Coordinator in Multifamily Housing Program. On-site services provided may include education, recreation, nutrition, and health-related activities; assists in planning, developing, and implementing new services. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

Essential Functions

  • Provides general case management and information / referrals
  • Arranges for needs assessment of residents
  • Develops individual service plans for residents that include goals and strategies for achievement; schedules regular follow-up meetings to assess progress
  • Maintains all HUD Standards for Service Coordinator in Multifamily Housing Program
  • Prepares and maintains required statistics and reports
  • Oversees and guides provision of services and activities including establishing and maintaining relationships with subcontractors and other agencies
  • Develops and presents educational workshops on issues of importance for residents and the larger community
  • Assists with development of systems and procedures related to client services, and creates supporting documentation and manuals as needed
  • Coordinates volunteers
  • Facilitates and attends staff meetings, staff development training and advisory board meetings as required
  • Transports residents for special events or on an as needed basis
  • Assists with fundraising activities that benefit senior programs and services
  • May drive agency or personal vehicle on company business

Minimum Qualifications

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • High School / GED
  • 2 years human service or related experience
  • Bilingual, English / Spanish written and verbal
  • Excellent data entry skills (speed and accuracy)
  • Proven skills utilizing Microsoft Office Suite
  • Excellent organizational skills
  • Proven strong communication skills oral and written communications are clear and concise using appropriate style, grammar and tone
  • TB skin test and drug screening
  • Obtain State of Arizona Level One Fingerprint Clearance Card
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle division 39 months Motor Vehicle Report

  • Additional years of directly related experience
