Working with the PATH center staff, you’ll coordinate services for our WICY, including medical case management, social work, psychotherapy, testing and counseling, and other support services to ensure optimal access to health care for our multicultural community. In this role, you’ll:

  • Conduct intake assessments and continual reassessment of patients (HIV+ or at-risk youth, children, pregnant mothers, and families)
  • Manage a caseload of 60+ high-needs HIV-positive patients providing intensive case management and psychotherapy services to women, transwomen, children, and adolescents/young adults
  • Develop care/service plans and ensure effective multidisciplinary service delivery
  • Provide follow-up care, referrals, and patient advocacy, including linkage to community-based agencies with pediatric, adolescent or family care programs
  • Work closely with the management team to determine patient and family needs, strengths, and barriers to care
  • Visit HIV+ mothers on the hospital floor and provide social work discharge services
  • Participate in administrative grant activities, monthly staff meetings, program promotion and outreach, CQI initiatives and represent PATH on external committees
  • Provide HIV testing, counseling and social work services to other ambulatory care sites as needed

To qualify, you should possess:

  • A Master’s degree in Social Work or Mental Health Counseling
  • 2 years’ experience working in an HIV clinic serving women, infants, children, and youth
  • Current NYS Licensed Master’s Social Worker or Licensed Mental Health Counselor; unlicensed candidates must obtain NYS licensure within one year of hire
  • Excellent written and verbal communication skills
  • An understanding of social, emotional, and mental health issues that impact HIV awareness and care
  • PC proficiency, preferably in Electronic Medical Record
