Requirements of Social Service Designee:
- Current social service designee certification or licensed social worker.
- Preferred minimum one-year experience in working long term care
- Excellent interpersonal skills, written and verbal communication skills
- Basic computer skills
Job Duties include:
- Being available and maintaining supportive relationships with residents and families.
- Coordinating with the interdisciplinary team to develop individualized psychosocial resident care plans.
- Accurately documenting pertinent behavioral and psychosocial information in progress notes.
- Reviewing resident advance directives on a routine basis.
- Completion of psychosocial assessments/reviews
- Providing education and resources to residents regarding advance directives, resident rights and Medicaid/Medicare services.

Pay: $15.00 - $21.00 per hour
