Our skilled nursing facility is seeking a Certified Social Worker to work with residents and help us deliver the best possible quality of life.
Responsibilities:
- Advocate for resident and families
- Uphold the rights of each individual resident
- Ensure psychological needs are met for each resident
- Function as part of the interdisciplinary team working toward the best interest of each individual resident
Duties include but are not limited to:
- Meet with resident and family following admission
- Responsible for coordinating the following referrals: psychiatric, psychological, vision, dental, and following up with the services during resident’s stay.
- On-going input of MDS and charting
- Formulate behavioral intervention strategies with resident, family and staff as needed. Intervene with resident, family and staff to work toward a solution for behavioral issues.
- Provide counsel and resources focused on social/emotional resources, adjustment issues, separation loss, dying and death to resident’s, families and occasionally staff.
- Work closely with RN supervisors and all members of the care plan team to ensure optimal care for residents.
- Facilitate care plan meeting every week. Invite residents and families, assess for change in mood or behavior, present assessments to resident and families and address their issues in a structured format.
- Discharge planning to ensure successful transition back into the community by utilizing financial, legal, mental health, meal programs, personal response systems, home health agencies and other community resources as needed.
- Work through court system and legal system to arrange psychiatric commitments as needed.
- On-going education of resident, families and staff as needed.
- Assist with early voting procedures and ballot by mail as needed.
- Assist Social Work Manager with development of policies and procedures for department.
- Other duties as assigned

Current and unencumbered social worker license- At least 2 years’ experience in a long-term care setting
