We are currently seeking a full-time Social Worker. Prior long-term care/short-term rehab experience preferred.

Responsibilities:

• Complete social histories, care plans, MDS assessments, trauma care assessments, psych referrals and discharge planning on a short-term rehab unit. Social Worker is responsible for 32+ Residents.
• Coordinate discharge planning for short term rehab unit including but not limited to finding alternative placement, setting up resources, arranging home health/ caregiver services, scheduling MD appts, ordering medical equipment (O2, nebulizers, accu-check machine, etc, wound vac, tube feeds), and home dialysis clinic.
• Referrals to and coordinate Hosparus Services
• Facilitate care plan meetings between family and Interdisciplinary team.
• Primary point of contact for Resident/family and the facility
• Issue Notice of Discharge and processing all appeal documentation.
• Present thorough knowledge of Resident and discharge plan in weekly Interdisciplinary Team Meeting
• Keep accurate and up to date documentation in Electronic Medical Record
• Ability to multi-task, prioritize and professionally interact with other departments
• Communicate with residents and family on an on-going basis regarding concerns, care, discharging planning, etc.
• Provide insurance updates and communicate Resident status to insurance companies
• Will also be Weekend day Manager on Duty several times throughout the year.
• All other duties as assigned.
