- Counseling residents and family members; facilitate meetings
- Complete Admissions, Psycho-social Assessments and Advanced Directives
- Make referrals for health services and coordinate discharge planning
- Completing all required documentation for medical records
- Must have at least 2 years of experience working as a Social Worker.
- Masters Degree in Social Work (MSW License is not required)
- Nursing or Rehabilitation Facility experience is a MUST!
- Should have a good working knowledge of available social services and other community resources in the area
- Must have excellent communication and social skills
- Self- directed but team-oriented
