The Lodge at Mills River Health & Rehabilitation is seeking an experienced Social Worker to join our team! You will work alongside fellow caregivers to provide high quality patient care.
Responsibilities:
- Assess nature of patients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans/care plans
- Determine appropriate milestones to gauge patient progress
- Communicate patient updates to all relevant parties

- Maintain accurate documentation
