Social Worker - Behavioral Health Patient Liaison
Job description
ESSENTIAL FUNCTIONS:
1. Planning, managing, and providing social servicesto ETCHSI patients in medical and mental health settings.
2. Providing treatment to individuals with mental health/substance use disorders, as well as various behavioral and emotional disturbances
3. Evaluating crisis situations and apply appropriate interventions.
4. Actively participating in meetings that support both the PCMH and integrated behavioral health care model to provide comprehensive care for clients.
5. Actively participating with patient educators to facilitate and incorporate elements of social service and PCMH best practices into the staff lead management of chronic diseases.
6. Assisting in the detection of ”at risk” patients and development of plans to prevent further psychological or physical deterioration.
7. Provision of social services as credentialed and within the staffs level of competence.
8. Maintaining liaison with area hospitals and coordinating and participating in social service and outreach activities.
9. Providing assistance in developing social services programs and resources.
10. Providing consultation on various social aspects of procedures, policies, and service to volunteers, medical staff.
11. Conducting staff training on best practices utilizing social work techniques.
12. Recognizing the role of client in the treatment planning.
13. Participating in general department meetings, treatment team, and case supervision meetings.
14. If bilingual, providing Spanish translation between non-Spanish speaking staff and Spanish speaking patients as requested.
15. Performing all other duties as assigned.
16. Coordinating and facilitating groups for identified patient populations.
EDUCATION:
Bachelor’s Degree in Social Work or related field.
EXPERIENCE:
Previous mental health experience preferred.
LICENSURE:
None required.
