Job Summary:

The Social Worker helps clients/patients who face a major illness, disability or life-threatening disease, and assist patients in coping with the health issues. Work involves acting as a member of a multidisciplinary team in serving patients and families. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors. Help with data collection and entry, completion of forms such as disability, and acts as resource to the community.

Essential Functions:

  • Attend and testify at court proceedings.
  • Maintains family contact regarding client adjustments.
  • Make assessments and provide appropriate treatment.
  • Actively participates in team review of records and cases.
  • Provides psychosocial support to patients and their families.
  • Participates in agency and community planning and education.
  • Develops and maintains client case records, plans, and reports.
  • Assess patients and/or family s needs and eligibility for services.
  • Assists patients to cope with chronic, acute, or terminal illnesses.
  • Develops and implements programs to address health and mental issues.
  • Offers information and counseling support to service patients and their families.
  • Participates in planning treatment programs, dispositional and discharge plans.
  • Maintains professional relationships with service users, acting as guides and advocates.
  • Ability to establish and maintain effective working relationships within the agency and community.
  • Undertakes and writes up assessments with medical staff to meet specified standards and timescales.
  • Advises family caregivers, counsels patients and help plan for patient s needs after discharge from hospital.
  • Works with clients, families and appropriate community agencies to coordinate and facilitate services for clients.
  • Provides selected agency services to identify caseload; refer clients and families to other services as appropriate.
  • Pertains to laws, regulations, and practices to federal and state human service programs and/or domestic violence programming.

Job Requirements:

  • Master s Degree in Social Work preferred accredited by CSWE.
  • A bachelor s degree in social work required to be accredited by CSWE.
  • Experience in psychology, sociology and related fields required.
  • CSW Certified Social Worker; LSW Licensed Social Worker; or LCSW Licensed Clinical Social Worker or the Diploma in Clinical Social Work credentials required.
  • Continuing Education Credits are required for each renewal period.

Tuesday, Wednesday, and Thursday 8:00am - 4:30pm
