As an integral Care Team member, Tap360 TCSs support primary care clinical teams by managing referrals and integrated clinical services for patients including preventive care, acute hospital care, outpatient diagnostic services, therapeutic services, post-acute care and specialty services.

Major Responsibilities:
- Serve as a liaison between providers, healthcare centers and hospitals.
- Collect, track and integrate patient information
- Manage referrals, schedule appointments, coordinate transition of care and ensure accurate communications
- Recordkeeping
Minimum Required Qualifications:
- High School Diploma or equivalent AND at least three years of experience work in a healthcare OR a post-high school course/certificate in a health-related area
OR
- Associate degree AND at least two years of experience in healthcare
Desired Qualifications:
- Bilingual
- At least one year of experience in care coordination or outpatient clinical experience
- Experience using EMR systems (e.g., athenaPractice, Centricity, Epic, Praxis, Meditech, Cerner, etc.)
