Aventura at Carriage Inn is seeking a full time Speech-Language Pathologist (CCC or CFY) for a skilled nursing position in Dayton, OH. Position will also cover a sister facility in Trotwood, OH. While these are medium size facilities, the rehab departments run busy programs and can easily support a full time caseload. This unique, in-house position offers an exciting opportunity to be part of a team of dedicated health care professionals that pride themselves in providing quality patient-focused care. Our Rehab Department has an excellent reputation in the community for providing rehabilitation services. MENTORING program available for new grads or those new to the SNF setting!

Duties Include:

• Evaluate Residents conditions for swallowing or cognitive deficits
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Help Residents through their tasks and monitor their performance
• Maintain Resident confidence & protects facility by keeping information confidential
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort
• Perform Daily Documentation
