We are seeking a Case Manager/Substance Abuse Counselor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Seventy-four units will be allocated for residents referred by the New York City HIV/AIDS Services Administration (HASA) and forty-nine apartments will be for families referred by the New York City Housing Authority (NYCHA).
The Case Manager/Substance Abuse Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills and an understanding of working with the HIV/AIDS population.

The Ideal Candidate:
- You have a Bachelor’s Degree in Social Work or a relevant field required
- You have a Credentialed Alcohol and Substance Abuse Counselor(CASAC) certification or a License in Clinical Social Work (LCSW) is required
- The ideal candidate must have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness
- You have at least 3 years of experience in substance abuse counseling
- You are knowledgeable of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues
- You have solid computer skills – internet, Microsoft Office, and charting database
- You have strong organizational, writing, and communication skills
- Spanish speaking is a plus, but not necessary
Key Responsibilities:
- Maintain a caseload and provide regular case management services to these individuals
- Maintain resident records in compliance with agency and funding standards
- Conduct and review assessments
- Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
- Monitor and ensure residents are linked to healthcare providers
- Provide counseling and crisis intervention as necessary
- Assist residents in securing and maintaining benefits
- Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
- Facilitate groups and activities that promote health and independent living skills development
- Monitor and assist residents with rent payments
- Conduct regular unit inspections
- Compile resources on substance abuse
- Educate residents and staff about approaches to managing addiction
- Oversee recovery readiness, relapse prevention, and recovery-oriented services
- Organize sober recreational activities
- Attend ongoing training to enhance the services provided to the residents
- Assist with the completion of HASA’s weekly and monthly reports as well as any other relevant reports
- Other duties as assigned
