Position: Substance Use Specialist (CASAC or CASACT-T) ACT Team
Location: Mt Vernon, NY
Hours: 35
Pay Rate: $27.47-$32.97 Dependent Upon Education and Licensure
$5000 Sign-On Bonus (Payable according to company policy)
Position Summary: The Substance Use Specialist is part of an interdisciplinary team of specialized team members that collaborate and plan daily for participants that have chronic mental health conditions, and some participants with a dual diagnosis of chemical dependencies. The ACT Team Substance Abuse Specialist provides direct substance abuse counseling to participants. 80% of the services are provided in the community. The substance abuse specialist will utilize evidence-based practices and measurable goals to determine stage of change and progress made. Strong engagement, teamwork, assessment, treatment planning, advocacy, follow up, time management, and documentation skills required.
Responsibilities:
  • Support a high-performing culture aligned with TGCW core values and B.A.S.I.C Tenets: Balance, Accountability, Satisfaction, Inclusion, and Communication.
  • Assists with the development of the social work assessment of patients/families, placing special emphasis on substance abuse/usage issues.
  • Assists in the development of treatment plans and goals based on a diagnostic assessment of the presenting problems and historical information, the establishment of long-term goals and administration of follow-up procedures.
  • Provides ongoing substance abuse services and/or collaborates with and refers to appropriate agencies.
  • Conducts individual and group counseling sessions.
  • Provides advocacy and assistance in obtaining entitlements and community services to patients/families and other caregivers.
  • Provides home visits to all clients as specified by the Programs level of service commitment and client’s needs.
  • Provides continuous evaluation of persons involved in treatment.
  • Assists staff, patients, families, and other caregivers to understand substance abuse issues, entitlements, and community resources.
  • Provides guidance, assistance, and referrals to enable participants to maintain a life free from drug use both during and after completion of treatment.
  • Assists clients with necessary by escorting them or arranging for escort to appointments and interviews.
  • Initiates and maintains written and verbal communications according TGCW policy.
  • Documents efforts with clients in chart and records detailed progress notes.
  • Participates in case conferences, staff meetings and training programs.
  • Act as a liaison with other community agencies.
  • Assumes responsibility for continued professional growth.
  • Participates in special projects and other duties as assigned.
Qualifications: Bachelor’s Degree Required. CASAC or CASAC-T Required. Master’s Preferred
Travel Requirements: 80% in the community providing services. Must have valid driver’s license.
Physical Requirements: Must be able to drive, walk and climb stairs of apartment buildings, carry a backpack with laptop and other office supplies need to complete service prevision and documentation.
