Supported Living Services Site Lead (Tucson)
Job description
The Site Lead provides managerial and direct support to a licensed or unlicensed setting under supervision of the Program Manager. Direct development, implementation and oversight of residential services of Easterseals Blake Foundation Supported Living Services (SLS) including management of program and program staff, compliance with all contractual, financial, and documentation requirements, as well as ensuring consistent and quality delivery of services. Duties include assessing, evaluating, and supervising services for individuals with complex behavioral health needs.
Essential Duties and Responsibilities:
- Review, participate in the interview and hiring process, supervise, and direct support staff including Behavioral Health Paraprofessionals (BHPP), and Behavioral Health Technicians (BHT)
- Responsible for ensuring delivery of activities and support services to participants at assigned site(s)
- Participate in training staff assigned to the site including documentation, entering service notes into the EHR, admissions and discharges, member services and care, site specific procedures, and monitoring staff activities
- Audit site staff documentation and adherence to procedural expectations; coordinate with Program Manager and Clinical Supervisor to provide additional training as needed based on audit outcomes
- Makes decisions on routine matters and consults Program Manager on more complex issues
- Coordinates and participates in the activities of a specialized program and maintains the approved staffing range to ensure quality care
- Responsible for scheduling, including on-call and call-out plans
- Responsible for monitoring use, acquiring, and adhering to the site’s budgeted amounts for household goods, food, cleaning supplies, and program supplies
- Facilitate clinical services using evidenced-based practices
- Communicates and interacts with residents on an individual and group basis, promoting individual choice, freedom, responsibility, and support
- Provides direct care to participants in accordance with the individual service and treatment plan, and ensures that staff assigned to the site are trained in the plans
- Promotes best practices in proactive strategies and trauma informed care to enhance member care
- Ensures standards of care are maintained and monitors members’ mental and physical health to enable appropriate intervention/prevention of problems
- Assists in monitoring expenditures and program allocations to meet current and future needs
- Monitor efficient controls to ensure the integrity of residential operating and member fund accounts
- Direct programmatic adherence to all contract requirements, including participation in acquisition and maintenance of all residential licensure requirements
- Conduct internal monitoring systems to ensure successful outcomes for all internal and external monitoring and reviews
- Assist with corrective action plans as necessary for contracting and compliance entities
- Timely assessment of all internal and external incident reporting and risk management assessment per policy
- Active participation in home visits, initial meetings, transition placements, high profile encounters, etc. to ensure appropriate placement and ongoing service delivery
- Effective communication and execution of policies, directives, and strategic plans as provided by Easter Seals Blake Foundation leadership
- Ensure contract deliverables including, but not limited to: staffing/ratio/range documentation to support billing of services, member progress reports, credential requirements, etc.
- Manages staff time and scheduling per site requirements
- Participates in staff and administrative meetings as well as in-service trainings
- Ensure SLS properties are maintained to ensure a safe, clean, and healthy environment for members and employees
- Ensure SLS equipment is maintained in a safe and operational manner at all times
- Availability to respond to critical needs of 24-hour residential services as assigned
- Periodic and routine programmatic inspections to ensure documentation and standardized practices comply with contract requirements, EBF policies, and other regulatory bodies
- Maintain positive communications and relations with employees, members, guardians, contract entities, authorities, and EBF
- Other duties as assigned
Education –
- High school diploma or GED required.
Experience –
- A minimum three (3) years of experience in the behavioral health field.
Regulatory –
- Minimum 21 years of age.
- Clean criminal background check and the ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card.
- Current valid, unrestricted driver’s license, proof of insurance, and a driving record that meets the standards of the agency.
- Proof of COVID-19 vaccination required and willingness to obtain future boosters if required by employer. Accommodation available.
