PATH is seeking qualified candidates sharing our passion for our mission to end homelessness to join our Homelessness Response Center team in San Diego as a System Navigator. ABOUT THE HRC
The Homelessness Response Center (HRC) is part of the City of San Diego Community Action Plan on Homelessness and supports efforts to achieve a coordinated, client-centric homelessness crisis response system within the city. As the primary service provider at the HRC, PATH's provides coordination and works in conjunction with outreach teams and other shelter providers to identify individuals who are prioritized for navigation services and assess their needs and preferences.
WHAT IS A SYSTEM NAVIGATOR?
The System Navigator plays a key role as the HRC by providing intensive, housing-focused system navigation services to program participants to support them through all stages of their journey to housing stability. Responsibilities of the System Navigator include:
- Responding to community requests for street outreach intervention and performing frequent outreach activities to connect and build rapport with individuals experiencing homelessness
- Working closely with clients to illuminate and address their housing barriers
- Providing intensive, person-centered system navigation, case management, and service coordination services to ensure clients have access and receive services necessary for housing stabilization
- Assisting clients with gathering documentation, securing mainstream benefits, and identifying appropriate and affordable housing options
- Ensuring clients are able to access services through provision of direct or indirect transportation
- Providing advocacy for clients including attending appointments, working with prospective landlords, and mediating disputes between homeless individuals and community members
- Collaborating with other PATH staff and community service providers to ensure seamless referral services
WHAT YOU BRING
We're looking for candidates that:
- Maintain and execute confidential information according to HIPAA and all other applicable standards
- Demonstrate strong skills with technology and computers as work tool, including proficiency with Microsoft Office software
- Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
- Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
PREFERRED QUALIFICATIONS
- Two (2) year's experience in a related field.
- Lived experience demonstrating successful recovery with issues related to homelessness, mental health, and/or substance use challenges which can be utilized to enhance effectiveness of services by informing service models and/or supporting clients through shared personal experiences.
- Proficiency with computers and technologies used for work outputs including:-Microsoft Office Software-Experience using a data entry system, customer relations management (CRM) system, and/or electronic health record (EHR)

MANDATORY REQUIREMENTS
For this role, candidates must:
- Be able and willing to work flexible hours which may include evenings or weekends
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver's license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
FLEXIBLE WORK
We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
