The Kid SpOt Center is seeking a full time energetic, career oriented Targeted Case Manager , who is passionate about kids and helping shape their future in.
The Kid SpOt Center is a pediatric clinic serving patients from birth to 21 years of age with various needs from developmental delays, Autism to Down Syndrome. Because we are dedicated to the success of each child, we offer multiple therapeutic services including: Physical, Occupational and Speech therapies, Positive Behavior Supports, Community Living Supports, Targeted Case Management and Mental Health Counseling. Job Duties:
- Complete comprehensive assessments of each participant's needs
- Provide quality care by developing and updating Plans of Care that are relevant and meaningful to the child and their family
- Provide quality, evidence based targeted case management interventions to address goals outlined on Plan of Care
- Monitor participant progress and demonstrate an ongoing understanding of participants needs and changes in those needs
- Collaborate with others directly related to participants to assist with identifying needs and care for the purpose of helping access services, identify supports, gather useful input regarding participant’s past and current functioning, symptoms, adherence and treatment information relevant to participants behavioral condition
- Complete all required documentation in a timely manner as outlined in policy (i.e. needs assessment, monthly summary, progress notes, billing sheet, etc.)
- Complete required updates for Progress Notes and Re-Assessments as outlined on Fusion to-do list and according to policy guidelines
- Provide minimum amount of monthly contacts that are appropriate for each participant and document those contacts appropriately
- Submit documents (i.e. SOAP notes, progress notes, monthly summary, etc.) to BSD Clinical Manager for review after completion
- Some travel required to Louisville clinic
Requirements:
- Possess a Bachelor’s degree in a human services related area (i.e. psychology, sociology, social work, special education, or other related fields)
- Minimum of one-year of direct, full-time experience in the area/population to be served
- Provide proof of completion or willingness to complete TCM Core, SED and SUD training within 6 months of employment
- Hold and maintain valid Adult, Child and Baby CPR/First Aid
- Valid Kentucky driver’s license and insurance
- Maintain annual TB test/assessment
