CommuniCare Health Services is recruiting for the key position of Lead Central Intake Specialist for our Home Health division in West Virginia.

The successful candidate for the Home Health Lead Central Intake Specialist will be an RN or LPN with Home Health experience. We are looking for a patient-focused, business-aligned self-starter with the ability to lead a growing team for scaled growth.

Key Responsibilities of the Lead Central Intake Specialist role include:

• Drive the central intake process for our Home Health division and support growth.
• Supervise Central Intake Specialists while working closely with our external marketing and nurse liaisons.
• Provide clients (prospective residents, their families, and referral sources) with program specific information.
• Evaluate and assess potential patients for clinical appropriateness.
• Help to supervise the admissions process and ensure proper admission documentation.
• Participate in sales and marketing team events and plans as needed.

To qualify for the Lead Central Intake Specialist role you must have:

• Valid state RN or LPN license
• Previous Home Health experience
• Sales, Intake, and Payor knowledge
• Solid computer skills, including Microsoft Office products
• Working knowledge of federal & state regulations and reimbursement (Managed Care, Medicare, and HMOs)
