JOB SUMMARY: The Team Lead provides clinical and administrative supervision to the Assertive Community Treatment (ACT) Program team members and to consumers on the ACT Team. The position works closely with the Director in providing intensive, integrated, rehabilitative, and crisis treatment to adults with serious and persistent mental illnesses and adheres to the principles and practices of the ACT Fidelity Model.

Supervisory Responsibility: Provides administrative and clinical supervision of 4-6 FTEs.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Provide an average of 2.5 hours of ACT services per day.
  • Participate in treatment team meetings as indicated.
  • Provide crisis assessment and intervention services as indicated.
  • Provide symptom assessment, management and individual supportive therapy as indicated.
  • Assist in the treatment of substance abuse for consumers with co-occurring addictive disorders.
  • Psychosocial rehabilitation and skill development.
  • Recommending and sometimes making decisions about the best course of action for a particular service user.
  • Provide support services to ACT team and consumers.
  • Conduct needs and risk assessment.
  • Provide resolution and referrals services for consumers.
  • Interpersonal social and interpersonal skill training.
  • Education, support and consultation to consumers’ families and/or their support system, which is directed exclusively to the well being and benefit of the consumer.
  • Provide four hours of individual and/or group supervision per month to assigned team members
  • Provide leadership and direction to ACT team members in areas of psychiatric care, management and operations in the absence of the ACT Director.
  • Provide on-call coverage on a rotating basis. Team Leader will provide consultation to ACT CSS on-call.
  • Review all daily service records for your team to ensure services meet ACT standards MHRS/Medicaid regulations for medical necessity. Service records must be reviewed as frequently as necessary to meet billing timelines.
  • Provide support and guidance to consumers and staff.
  • Develop and implement quality improvement activities for individual clinical staff members and the clinical aspects of the ACT team.
  • Enter all service activities into electronic records by the following business day from the date of service.
  • Attend clinical rounds and QI meetings as appropriate.

EDUCATION and EXPERIENCE:

  • Master’s Degree in Social Work, Counseling or Psychology from and accredited college or university.
  • Relevant license in appropriate discipline LGSW, LICSW, LGPC, LPC.
  • 5 years of relevant experience with at least 3 being in a mental health setting with a preference working with an ACT Team.
  • 1 year of previous supervisory experience.

SKILLS and COMPETENCIES:

  • Communication skills (written and verbal).
  • Skills in co-occurring disorders treatment.
