FUNCTIONAL PURPOSE
The purpose of this position is to meet the needs of Crossnore Communities for Children's youth with transitional services and for WS/FCS students transitioning from various education programs and support facilities/group homes. This role assists with educational advocacy and manages the transitional plan which includes pre/post reporting for Crossnore's at-risk youth. The employee is responsible for working with various school staff and supporting the family case workers and case managers in Crossnore's child welfare programs. In addition, the employee will use a wide range of local resources and placements to provide transitional support to students and families. This is a 12-month position.

Pay Grade: Certified Teacher Pay Schedule - 12 Month Position

DUTIES AND RESPONSIBILITIES:
- Participate in BID meetings and assist with enrollment of youth into schools.
- Collaborate with the WSFCS Social Worker for Foster Care.
- Educational Support Supervisor and Case managers in developing educational plans for each youth.
- Works with Education Support Supervisor to provides educational advocacy and support to family caseworkers and case managers in Crossnore's child welfare programs; including, but not limited to, supporting networking with school systems, advocating for educational accommodations, supporting educational transitions at admission and discharge, interpreting educational documentation, etc.
- Develop & manage Transition plans for students in transition.
- Attend Child and Family team meetings & EC/504 meetings for students who are transitioning.
- Coordinate necessary academic and non-academic interventions for each youth working in close collaboration with all available resources.
- Maintain data on youth interventions and progress.
- Collaborate with teachers to collect and analyze individual student tracker
- Performs other related duties as assigned.
- Ability to communicate effectively, orally, and in writing.
- Strong knowledge of instruction and assessment.
- Ability to analyze data from multiple sources.
- Strong instructional technology skills.
- Experience with online educational programs.
- Strong organizational skills.
- Strong problem-solving and critical thinking skills.
- Ability to work in a collaborative environment.
- Successful evaluations or work experience that demonstrate success in building and maintaining relationships.
- Knowledge of Trauma informed Care & Child Welfare Systems
- Minimum: Bachelor's degree in Education or relevant Social Science discipline PLUS one year of experience working directly with youth in the Child Welfare System in educational environments.
- Desirable: Bachelor's degree in Education or relevant Social Science discipline PLUS one year of experience working directly with you in the Child Welfare System in educational environments. Experience working with Title I & federal programs. Experience working with trauma-informed care and understanding of IEP's, 504's & BID's with knowledge of navigating the EC/504 system to meet the needs of the student.
