Transition Liaison - Maine DHHS

The Department of Health and Human Services (DHHS) provides supportive, preventative, protective, public health and intervention services that safeguard the public health and assist our customers in meeting their needs. The Office of Child and Family Services (OCFS) within DHHS is dedicated to helping create a future where all Maine children and families are safe, stable, happy, and healthy. The Office of Aging and Disability Services (OADS) within DHHS is responsible for ensuring the safety, support and well-being of adults throughout Maine.

Overview

The Department will be hiring four transition liaison positions, two housed at OCFS and two at OADS. The individuals in these positions will be responsible for learning and knowing their two systems and working together to cross-train and support children and adult case managers, school system personnel and families. These staff members would be supplementally trained and expert in transition related needs, timelines, and documentation. From an external support perspective, these liaisons would deliver the same types of support services.

Section 9817 of the American Rescue Plan Act of 2021 (ARP) provides states with a temporary 10 percentage point increase to the federal medical assistance percentage (FMAP) for Medicaid HCBS expenditures. The temporary funding must be used by March 31, 2024 to improve or enhance Medicaid HCBS. HCBS provides critical supports and services to thousands of older adults and individuals with disabilities in Maine, enabling them to live in their homes and communities, rather than in nursing homes and other institutions.

Duties and Deliverables

The Transition Liaison duties will include:

  • Identifying cohorts of transition-age youth, defined as individuals who are age 14 and receiving Children’s Behavioral Health Services (CBHS) services who will require specific transition planning
  • Transition case management support and implementation
  • Attending or supplementing IEP processes so that evaluations conducted in schools can suffice for OADS applications (e.g., within a two-year window of application)
  • Securing supplemental evaluations outside the school as needed, such as those examining adaptive capacity, which is often excluded from school-based evaluation
  • Working with case managers to develop transition plans that consider needs and expectations related to services and supports, such as vocational, health, behavioral health and other services
  • Monitoring/working with child and adult community case managers to enter relevant information into their respective case management systems
  • Triaging cases where family-members/caregivers act as case managers: for scenarios involving individuals without case managers, the liaisons could intervene, refer to case management, and try to help problem-solve in situations with family members/caregivers, but, because of resource demands and systems-level responsibilities, would not directly case manage such individuals
  • Facilitate access to state systems to ease application processes: Would provide connection to additional state systems as needed, such as ASPIRE, Vocational Rehabilitation (VR), EIS, Evergreen, MACWIS, Atrezzo, etc.
  • Training and education: within their region would deliver trainings to stakeholders such as school districts on Guardianship alternatives including supported decision making, Differences in available service intensity as individuals move from OCFS to OADS, and Differences in language, terminology, and service philosophy as individuals move from OCFS to OADS
  • Systems level reporting, quality assurance, quality improvement and monitoring: The 4 Transition Liaisons and Project Manager will gather (virtually) and identify trends in transition caseloads and report out on trends and outcomes to DHHS leadership
  • In partnership with DHHS leadership, staff will work with the OMS Policy Planner to pull and review reports of individuals approaching transition age in order to guide the scope of their work and forecast potential OADS capacity needs, and associated funding; this would enable smooth transitions, including transitions out of children’s residential settings

Qualifications:

Experience and Education

  • A Bachelor’s Degree in Public/Community Health, Health Administration, Health Sciences or Health Education; Public Administration, Government, Social Work, or a closely related field
  • Two (2) years professional experience in the planning, development, coordination and/or evaluation of health programs or services -OR- an equivalent six (6) year combination of education, training, and experience.

This is a full time 40-hours per week position

Preference will be given to individuals who have experience working with transition age youth with disabilities.
