The Purpose
To create a premier health care community by directing, overseeing and evaluating all clinical personnel in delivering resident centered quality care according to state/federal regulations and set forth policy & procedures while balancing cost vs care to meet the goals of Touchstone Communities through optimizing systems and effectively managing a stable clinical team with the appropriate skill sets.
Essential Job Functions:
Reviews, supports, mentors and guides the community nursing team members on systems, nursing standards and guidelines. Responsible for following the position agreement of the community DON role, as follows:
1. Responsible for the development of nursing team members within the Clinical department while maintaining performance standards of nursing practice.
2. Accountable for implementing and sustaining approved policies, programs and process guidelines.
3. Direct, evaluate and supervise delivery of resident care and initiate corrective action when identified.
4. Identify concerns/opportunities and develop actions plans, training and monitoring for compliance.
5. Assess resident care needs and assist in the development of individualized plans of resident care to include the initial 48 hour baseline care plan and review of care plan summary.
6. Accountable for bi-weekly QOL (at minimum) reviews conducted on all residents with falls, pressure ulcers, and/or weight loss/hydration, antipsychotic medications and re-hospitalizations.
7. Review, support, mentor and guide clinical nursing team members on systems, nursing standards and guidelines.
8. Identify concerns/opportunities and formulate actions plans, training and monitoring for compliance.
9. Responsible for following State and Federal guidelines for timely reporting ANE (Abuse, Neglect and Exploitation) allegations and initiating investigation and follow up of findings.
10. Responsible for documentation of concerns and/or grievances and provide a summary, findings and corrective action taken.
11. Review tracking and trending of systems/processes and develop action plans as needed.
12. Review and respond to weekly/monthly consultant (RD, RX, State QM visits) reports within seventy-two hours of receipt of consultant visit report.
13. Complete reports, plans of actions and/or monitoring tools prior to the community Skilled Division FOCUS calls or VLB Advance calls.
14. Implement action plans and monitoring tools in response to comprehensive review findings.
15. Perform weekly skin rounds on residents with identified wounds (weekly “eyes on” for new or worsening wounds and all pressure areas currently being treated within the community) and complete the weekly skin reports; cross reference and validate clinical documentation in PCC.
16. Conduct daily audit review of each PCC system during clinical meeting to include but not limited to:
a. Risk Management- Assume responsibility for analysis of incident and accident investigation reports to determine cause(s) and implement corrective action(s), when appropriate
b. 24 hour Summary – run report daily for look back and review for anything that flags
c. Physician Orders – daily telephone and monthly consolidated orders pending signatures and review,
d. Physician Order Listing – review orders written in last 24 hours
e. Clinical Dashboard review – Un-reviewed radiology or lab results, Interact, follow up on alerts and change of condition alerts.
f. Baseline Care Plans – Initiated within 48 hours and reviewed/signed by RN
g. In Progress UDA’s closed and locked.
17. Review Community reports, such as VIGOR, 5 Star and Casper Reports, and identify areas of opportunity in order to refine processes &/or implement corrective action and teaching opportunities to improve resident centered care and follow up if recommendations are requested from Regional Nurse.
18. Participation in the QAPI Program content with focus on comprehensive data-driven programs, systems of care, outcomes of care and quality of life to validate areas are being addressed and progress noted.
19. Participate in each assigned PIP Charter for QAPI and report results monthly to the QAPI Team.
20. Educate clinical staff to promote competency in care and delivery. Verify nursing team members annual skill validations are current.
21. Participates in all training as required including Relias, PCC, Point Right and company clinical program rollouts.
22. Participates in community on-boarding for orientation training to include utilizing discipline specific checklist to include competency skill check off document.
23. Review staffing and scheduling to acuity and review staffing PPD budget to actual to identify opportunities. Utilizing Smartlinx projection reports and labor management toolbox.
24. Validate chart review of all new admissions has been completed within 24 hours
25. Provide oversight for implementation of nursing rehabilitation and restorative programs.
26. Conduct or participate in resident care conferences or delegate to qualified individual.
27. Consistently operate nursing service department within the established budget guidelines.
28. Complete Relias American Association of Nurse Assessment Coordinators certification within 6 months of hire and complete renewals as required.
29. Complete State MDS LTCMI certification course through Texas State University with in first month of hire and renew per state regulatory requirements.
30. Consistently work cooperatively with residents, residents’ representatives, community team members, physicians, consultants and ancillary service providers.
31. Perform duties assigned in an effective, timely and professional manner.
32. When not assigned to a community as DON, under the direction of SVP of Clinical Services will participate with the regional team in areas of comprehensive reviews, support regional nurse in community visits, and participate in special projects.
STANDARDS:
Position-Specific:
1. Overall V.I.G.O.R. within 90% benchmark.
2. VIGOR review with regional nurse by 4th of each month and data posted to InTouch by the 5th of each month with drill downs to Regional Nurse, as requested on areas identified as concerns/no progression, by the 10th of the month.
3. Survey findings of no more than 5 F tags with a scope and severity of no greater than “E” and no substandard quality of care.
4. 5 Star Quality Measure rating 4 star or greater.
5. RTA’s company benchmarks.
6. QIPP (Quality Incentive Payment Program) requirements as set forth by State requirements.
QUALIFICATIONS:
- A current and valid Texas RN nursing license or a current and valid RN license from a Compact State is required. The RN may continue to practice under the former home state license and multistate licensure privilege during the processing of the nurse’s licensure application in the new home state for a period not to exceed thirty days.
- Prior leadership as a Director of Nursing in a skilled nursing community is required.
