Wellness Coordinators provide telephonic care coordination and assistance with locating needed clinical and community resources to ensure successful outcomes and maintain a high level of customer service. They are responsible for entering relevant data and clinical information into multiple platforms.

If you are located in the state of Ohio, you will have the flexibility to telecommute* (work from home) as you take on some tough challenges.

Location: Remote in the state of Ohio

Schedule: Monday through Friday 8:00 am- 5pm No Nights weekends or holidays .

Primary Responsibilities:

  • Outreach to members or providers who require additional assistance with complex situations
  • Close collaboration with ICT, other Optum functional teams
  • Assist with direct linkage to available providers and confirm successful linkage with an appropriate provider
  • Assists enrollees with understanding locating community support services and confirms successful linkage
  • Assist with provider searches when necessary to meet specific clinical needs, availability issues or geo-access issues
  • Outreach to members who require assistance navigating to an appropriate level of care or provider following an adverse benefit decision
  • Assist with other coordination of care needs with Optum BH staff
  • Demonstrate initiative in achieving individual, team, and organizational goals and objectives
  • Data collection in a provider facing role; input into multiple documentation systems
  • Other duties as assigned

Required Qualifications:

  • High School Diploma / GED (or higher) OR 4+ years of equivalent medical office\behavioral health experience
  • 2+ years of experience in Social Services or Behavioral Health field (i.e. hospital, managed care organization, or joint behavioral outpatient practice)
  • 1+ year experience with/ knowledge of the resources available, culture, and values in the community
  • Intermediate level of computer proficiency including strong data entry, utilizing Microsoft Office including Microsoft Word (able to create, edit, copy, save & send documents), Microsoft Excel (able to create, edit, copy, save & send spreadsheets, Microsoft Outlook (sending / receiving emails and calendar invites)
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) and is able to secure all company sensitive documents secure including Protected Health Information (PHI)
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:

  • Bachelor's degree in mental/behavioral health field
  • 1+ year managed care experience to include casework experience at hospitals, social service agencies or community mental health centers
  • Exposure to mental health and substance use disorder diagnoses and terminology
  • Healthcare experience
  • Prior experience with behavioral health environment
  • Prior work at home experience

  • Demonstrated effective organizational skills and scheduling skills as observed in day-to-day work
  • Excellent customer service skills
  • Excellent interpersonal and problem-solving skills

  • Must demonstrate flexibility, ability to follow through on all tasks in a timely fashion, good attention to detail and a willingness to learn

  • Able to handle sensitive issues with members and providers in a confidential manner
  • Demonstrates initiative in achieving individual, team, and organizational goals and objectives
  • Creative problem-solving skills with the ability to use community and network resources to meet members' needs
