Job description
Activity Coordinator plans and provides an ongoing program of activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well-being of each patient.Plans and provides activities appropriate to the needs of the patients on a daily basis; plans activities for evening and weekend times and coordinates with other staff to carry out plan; assesses patient needs and develops treatment plan accordingly; properly documents progress notes in electronic medical record.
Education: Must be willing to become a certified activity professional.
Experience: Placement or experience in mental health setting preferable.
Location: Good Samaritan Hospital - INDIANA ยท Samaritan Center - LaSalle Behavioral Health
Schedule: Part Time Permanent, Day Shift, 8am-5pm with rotating weekends
