ProMedica Palliative and Hospice Care formerly Heartland Hospice is a part of the ProMedica family, a leading provider of home health care, hospice care, skilled nursing, memory care and post-acute care. We are currently searching for an experienced full-time health care professional to provide service and support to our referral sources, community members, patients and families. This role will involve working in the Buffalo Grove, IL office Monday-Friday days. The role also involves limited after-hours on call.The Admission Coordinator is an internal customer service position that is responsible for coordinating the referral-to-admission process for palliative and hospice patients. They provide administrative support to the admission team to ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency growth and profitability is maximized.
