Description
Center for Community Resources, Inc. (CCR) is currently seeking professional and self-motivated Service Coordinators to serve Pennsylvanians in the Community Health Choices waiver. CCR is NCQA accredited and has been successfully managing the roll-out of Community Health Choices in zone 1. We are looking to expand our team with immediate full-time openings.
Essential Functions
Center for Community Resources, Inc. (CCR) is currently seeking professional and self-motivated Service Coordinators to serve Pennsylvanians in the Community Health Choices waiver. CCR is NCQA accredited and has been successfully managing the roll-out of Community Health Choices in zone 1. We are looking to expand our team with immediate full-time openings.
Essential Functions
- Use a person-centered support planning approach and team process to develop the participant’s care plan in order to meet their needs in the least restrictive manner possible.
- Coordinate person-centered support planning with assigned Managed Care Organizations (MCOs) to ensure continuity of care.
- Maintain accurate records, documentation and compliance with standards.
- Operate independently through self-direction and accountability.
Responsibilities
- Monitor health and safety as well as service utilization for each participant following a pre-determined schedule.
- Complete InterRAI assessments as mandated by the MCO.
- Function and communicate within a multi-disciplinary team.
- Provide telephonic and in home assessments as requested by the participant or required by MCOs.

Qualifications
Bachelor’s degree including successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services. At least three (3) three years of experience in a social service or health care related setting. ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire.
