Salary Range: $110,000.00 per year

By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company.

A Internal Medicine Group has an opportunity for a Full Time and a Part Time Advanced Practice Registerd Nurse (APRN) or Physician Assistant (PA) to work with patients at home in the Meriden area. This is an excellent opportunity for those that enjoy a flexible work schedule,working autonomously, and accelerate in a pay for performance environment.

The APRN or PA will travel to patients homes to provide care.

Essential Function

• Visiting patients in the home to assess, care for and and create a care plan
• Leading the Home Health Clinical Team on patient care in the home
• Create a care plan for the patient
• Utilize EHR, Epic to update patient chart
• Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
• Analyze patients medical history, medication allergies, physical condition and examination to determine best care
• Diagnose medical condition
• Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
• Directs and coordinates the patient care activities of nursing and support staff as required.
• Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards

Education and Experience

APRN

• The Advanced Practice Registered Nurse is masters or doctorate prepared
• Certified Nationally as a Nurse Practitioner through AANP or ANCC .
• The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.
• DEA, and Ct controlled substance license
• Experience working with elderly patient population
• Experience working with EHR

PA

• Physician Assistant Certification
• State License
• Any years of experience
• Experience working with EHR
