Summary:
The Assistant Director of Penn Center for Community Health Workers (Center) provides operational leadership for the IMPaCT Community Health Worker (CHW) program within Penn Medicine and the Philadelphia region. (S)he oversees management and CHW staff and drives quantitative and qualitative outcomes attainment for a core portion of the Center's work. The Assistant Director discovers and leads new opportunities and triages requests to integrate CHWs into clinics and hospitals.
Responsibilities:
Oversees operations and metrics at a large clinical site (e.g., inpatient and outpatient programs present) or program level (e.g., all inpatient or outpatient operations).- Supervises a staff of Managers/Sr. Managers and CHWs to ensure continued quality of outcomes for about 1,000 patients each year.
- Analyzes data on a regular basis and identifies opportunities for improvement. Develops and implements corrective action plans with staff.
- Serves as first point of contact to evaluate opportunities for expansion into new sites.
- Represents Center in major health system initiatives that address the needs of low-income patients.
Credentials:
Education or Equivalent Experience:
Bachelor of Arts or Science (Preferred)- 8+ years Experience in a management role with a track record of achieving program outcomes and developing high-performing staff5+ years Leadership experience in organizations focused on improving the lives of low-income individuals
