The Baylor RN - Registered Nurse for our hospice program works 32 hours and is paid for 40, the schedule is 8 hours on Friday and 12 hours each day on Saturday and Sunday.
Responsibilities
- Will perform scheduled visits and admissions for our Norwalk and Orange territories.
- Provides one-on-one skilled hospice nursing care.
- Responsible for the coordination of all patient care services.
- Will support the philosophy, goals, and culture of Constellation Health Services
- Provides direct supervision of field staff and coordinates activities of the health agency to provide quality care according to established best practices, federal and state regulations and Agency policies.
- The Baylor RN reports to the Director of Hospice Services.
Qualifications
- Registered Nurse - RN with current licensure to practice professional nursing in the state of Connecticut.
- Must have some previous experience in the hospice/home care setting.
- Complies with accepted professional standards and practice.
- Has excellent observation, good nursing judgment and communication skills.
- Understands hospice philosophy and issues of death/dying.
- Understands principles of pain/symptom management.
- Demonstrates excellent observation, verbal, and written communication skills.
