Summary: Position responsible for direct, basic and supportive care to consumers within a program.

Essential Duties and Responsibilities include the following. Other duties may be assigned to meet Consumer and agency needs. • Provides training, health monitoring, assistance with activities of daily living and transportation for disabled adults. • Provide oversight and intervention to consumers that are experiencing a major behavioral health crisis. • Serves as a member of the residential/day support services team and attends all designated meetings. • Maintains daily logs and records of patient care, condition, progress or problems to report and discuss observations with supervisor or Service Coordinator. • Monitors consumer safety needs, including physical, medical, behavioral and psychiatric. • Monitors consumers to ensure their safety by observing health and vital signs, including blood pressure, pulse, temperature and respiration. Documents consumer functional and dysfunctional behavior according to individual program plan. • Maintains facility to ensure sanitary conditions. • Assists with cooking, laundry and other general household activities necessary to maintain sanitary conditions and general housekeeping upkeep. • Accompany consumer to doctor’s offices and on other trips outside the home, providing transportation, assistance and companionship. • Comply with federal, state and company policies, procedures and regulations • Maintains confidentiality of all information and complies with HIPAA regulations. • Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior. • Other duties may be assigned to meet consumer and agency needs.

Qualification Requirements: • Must work well in a team environment • Work with staff, consumers and others in a professional, cooperative, ethical, respectful and effective manner. Education and/or Experience: Three (3) years experience. Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen. Must have valid driver’s license and current auto insurance.

Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Mobility and ability to stand and walk for long periods, bend, reach and lift up to 50 pounds is required in this position

Pay: $10.00 per hour
