• Base hourly rate of $16.85 to $20.23 per hour; based on previous relevant experience

Responsibilities of a Behavioral Health Coordinator

• Provide service coordination for you under the SED Wavier to assure that an integrated, comprehensive plan of care, which includes family members, natural supports, and relevant community providers, is developed, and maintained.
• Advocated for and with clients as they seek to achieve their goals and overcome barriers
• Provide face-to-face, phone, and written community with parents/guardians, other service agencies, schools, and medical services to assist meeting client/family needs.
• Provide assertive outreach directed toward engaging and maintaining client relationships as well as relationships with caregivers

Qualifications of a Behavioral Health Coordinator

• Bachelor’s degree required. Degree in Social Work, Psychology, or Human Service field preferred
• Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered
• One year volunteer or paid experience working with individuals in the community
• Valid Driver's License and proof of auto insurance
