Job Title: Behavioral Health Case Manager - Lawrenceville, GA.
Shift: First – Full-Time (40 hours per week on average)
Job Hours: 7:00 AM – 4:00 PM

Job Description:
The Behavioral Health Case Manager provides intensive case management services for clients with severe and persistent behavioral health disorders. Typically, the goal is prevention of hospitalization or other out-of-home placements. The case manager provides services primarily in an Adult Day Services Program.
The Behavioral Health Counselor primarily facilitates skill building groups in addition to conducting face-to-face comprehensive assessments to identify problems and service needs and assists with writing individual service plans that meet Medicaid, insurer, CARF, and other required standards for diagnoses, content, justifications for treatment, evidence of client input, and treatment time frames. Writes daily progress notes that are clear, concise, complete, grammatically correct, timely, properly coded and relate directly to the treatment outcomes described in the service plan. Secures or provides necessary signatures required to secure authorization for services. Follows all Medicaid, state, insurer, and other regulations related to the credentials required to provide authorized services. Provides and bills only those client services that are authorized in the service plan.
- Monitors client progress and compliance with the service plan.
- Assembles, analyzes, and interprets new/updated information about the client in order to reassess treatment needs.
- Identifies unmet needs, reviews available resources, and assists with modifying the treatment plan as indicated.
- Provides service linking and monitoring.
- Establishes linkages and coordination of services with community agencies.
- Develops and maintains cooperative working relationships with community service providers. Informs clients and families of the availability of community advocacy and support groups.
- Provides transportation for clients on community outings to offer opportunities to teach and practice skills taught in groups that are related to independent living, employment, socialization and communication in a natural setting.
- Other critical services provided include symptom management, behavior management planning, teaching of relevant skills, supportive community-based interventions, and crisis intervention services.
- The Behavioral Health Clinical Case Manager participates in interdisciplinary case staffing and staff meetings.
Minimum Qualifications:
Master's degree in a related field from an accredited college or university OR Bachelor's degree in a related field from an accredited college or university AND Two years of related experience OR One year of experience at the lower level Behavioral Hlth Counselor 1 (HCP100) or position equivalent. Note: Some positions may require certification or licensure.
Preferred Qualifications:
Master's degree from an accredited college or university AND Two years of related experience OR Bachelor's degree from an accredited college or university in related field AND Four years of related experience OR One year of experience at the lower level Behavioral Hlth Counselor 2 (HCP101) or position equivalent. Note: Some positions may require certification or licensure.
