San Luis Valley Health is seeking a full-time Behavioral Health Consultant to provide behavioral health support in the San Luis Valley Health's Medical Office Building, Conejos County Hospital, emergency department, hospital floor, and the labor and delivery department. This support will include providing preventative screenings, safety assessments and planning, crisis intervention, psychological first aid, making referrals and connecting with resources.

What You’ll Need:
  • Bachelor’s degree in Psychology or related field required
  • Master’s degree in field of mental health (such as counseling or social work) in process, required
  • Trained in crisis and emergency intervention, preferred
  • BLS Certification required
  • Registered with DORA as Unlicensed Psychotherapist, or per licensure, required
  • Experience in Substance Use Disorder and recovery, preferred
  • Experience with screening, assessment, and mental health counseling, preferred
  • Strong communication skills – clear and professional communication with patients, providers, colleagues, and administration
  • Comfortable with being a self-starter and taking initiative to respond to the needs of the team – This is a new role with room for creativity and development!
  • Comfortable working in a fast-paced medical setting
  • Skilled at multi-tasking, organization, and attention to detail
  • Competent in using basic technology

What You’ll Do
  • Work with a multidisciplinary team in multiple locations, including RMC and CCH hospital floors and Emergency Departments, Labor and Delivery, and clinics, to address psychological and social needs of patients
    • This current position primarily focuses on hospital interventions
  • Conduct psychosocial assessments for patients to support ongoing needs, discharge planning, and the continuum of care post-discharge, and to determine ongoing behavioral health needs and drive treatment planning.
  • Refer/connect patients with community resources
  • Provide screenings and brief interventions to patients regarding ongoing substance use, crisis intervention, or behaviors that are in need of further care, placement review, or prevent readmissions
  • Conduct safety assessment and safety planning, and coordinate emergency response
  • Help facilitate communication between patient and their multiple providers
  • Provide psychoeducation to patients and their families
  • Support staff with education and response to psychosocial needs of population
  • Support Discharge Planner in higher-acuity discharges
