We are currently seeking a mission-driven Behavioral Health Integrated Clinician to be an integral part of our behavioral health and medical team. The clinician in this role is responsible for delivery of integrated behavioral health within the health center. Providing consultations, therapeutic evaluations and interventions utilizing a variety of short-term treatment approaches in working with a culturally diverse patient population.

RESPONSIBILITIES

1. 60%

Clinical Care-

Integrated Care

- Meet with patients and families of all ages to triage patients’ needs and arrange appropriate services for patients either with the clinic’s Behavioral Health outpatient team or make referrals to outside agencies.

- Provide Behavioral Health services in the primary care setting; more specifically can provide initial assessment, determine appropriate level of care, engage in crisis intervention, psychoeducation and/or short-term intervention for both primary care patients and patients seeking treatment as a walk-in.

- Performs assessments to include identification and evaluation of complex risk factors and assigns diagnosis (when appropriate). Develops and implements short term interventions.

- Respond to same day visits, warm hands offs, consultation, or crisis related interventions.

- Provides consultation to health center medical staff, affiliated programs and/or the community as requested. Plans and conducts formal didactic teaching and in-service education programs for primary care and behavioral health staff.

- Respond, log, and track all incoming BH referrals and process accordingly for initial visits and follow-up to assess level of readiness to commit or engage in long term treatment.

- Respond to urgent or emergent crisis as requested by the medical staff (51-A or Section 12).

- Records assessments and interventions in patient medical records according to established clinical documentation standards. Prepare patient reports, correspondence, referrals and other materials for community agencies, schools, legal proceedings, and others as needed.

Outpatient therapy:

- Perform short and long term outpatient therapy to a small caseload utilizing the appropriate clinical modality based on diagnoses and risk

- Create treatment plans and refer clients to appropriate services based on assessment and treatment plan needs.

- provide ongoing treatment, assess patient’s ongoing clinic needs and progress, and provide consultation and coordinate care with other designated team members.

- Prepare appropriate transfer, discharge plans, and assist clients in a therapeutic transition from services.

2. 40%

Case Management/Care Coordination

- Provide patient with external community resources pertaining to BH services beyond the scope of practice at MCHC.

- Work with Population Health team to engage in patient outreach and Quality Improvement initiatives

- Work collaboratively with specialty teams (OBAT, HIV, OBGYN, etc.) to provide clinical case management, outreach, follow up, and linkages to services as indicated for patients’ care plans.

- Attend Behavioral Health team meetings, routine staff meetings, and other team meetings for integrated care.

- Manage BH wait list

- Assist BH providers with obtaining signed consents and ROIS

- Perform other duties as required by the position or as assigned by the BH Director. SUMMARY OF QUALIFICATIONS AND REQUIREMENTS:

- Master’s level Social work or Counseling/Psychology degree.

- Licensed preferred: LCSW, LICSW, LMHC, and/or LMFT. Current, valid, and unrestricted licensure to practice in MA.

- Minimum of one to two years relevant clinical experience.

- Bilingual/bicultural in Haitian Creole or Spanish strongly preferred.

- Good communication skills with staff and patients.

- Experience in working with diverse cultures preferred.

- Excel, outlook and EMR experience preferred
