POSITION SUMMARY:

The purpose of the Behavioral Health Supervisor position is to facilitate the appropriate delivery of healthcare services for patients with mental health diagnosis assigned to the Aunt Martha’s Health Care Network. The Behavioral Health Supervisor is expected to coordinate and collaborate with other programs and agencies working with assigned Aunt Martha’s patients, including health center staff. The Behavioral Health Supervisor will also manage and monitor TOC patients to ensure appropriate follow up including adhering to the TOC process and scheduling timely primary care visits and behavioral health visits to prevent 30 day readmission and patient adherence to discharge care plan. This position will report directly to the V.P. of Care Coordination. The BH Supervisor will be expected to provide supervision, direction and coaching to the behavioral health consultants, behavioral health care coordinators and behavioral health support staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Master EHR/ Athena system to efficiently schedule appointments, scan records or hospital discharge paperwork, locate records, ect…
• Appropriately document data entry and patient contact information in Athena and other necessary portals
• Participate in required meetings, workshops and trainings related to Behavioral Health and the TOC process
• Function as a liaison between behavioral health and primary care
• Participate in all related patient care team meetings, case consultation and clinic huddles
• Ensure that all patients served experience a high level of customer satisfaction and coordinated care
• Provide mental health education and substance abuse education to patients
• Provide case management , community linkage to necessary resources and patient advocacy
• Facilitate trainings to health center staff in support of the integration between primary care, behavioral health, and substance abuse treatment.
• Provide direction and coaching to behavioral health consultants and BH care coordinators
• Complete necessary assessments including but not limited to evidence-based screening tools (i.e., SBIRT, PHQ-2 &9, GAD7, Vanderbilt, etc.)
• Understanding of Chronic medical conditions and the impact on the patient
• Collect data pertaining to patient delivery of care and manage Database that includes outcome data
• Provide therapeutic services to patients in need of behavioral health care needs that have recently been discharged from the hospital
• Respond to patients, peers and referring agencies (specialists, hospital staff) in a positive and timely manner. Communication and inquiries may be by telephone, mail, fax & email.
• Collaborate with health plans, specialists, hospitals, and physicians to ensure complete and total patient care requirements.
• Ability to communicate effectively with physicians, providers and clinic managers
• Monitor MCO portal Dashboard for daily TOC patients and follow TOC process
• Provide appropriate follow up from MCO and ACO notifications for TOC patients to ensure timely transition to primary care
• Establish communication schedule both face to face and via telephone contact with hospital care managers to review patient information, verify hospitalization and receive discharge summary care plan
• Provide face to face patient follow up at hospital for high risk/ complex patients to engage into practice and verify contact information
• Monitor patient progress and provide continuous communication with medical home care manager and care team including PCP
• Follow agency policies, procedures and guidelines.
• Perform all other job duties and responsibilities as assigned.

• Access to transportation is necessary as position requires travel between clinic sites, off-site meetings and trainings are required
• Ability to be flexible with scheduled hours to meet patient needs and clinic operation needs for Behavioral health services

POSITION REQUIREMENTS/EDUCATION:

Education:

• Master’s Degree in Social Work or related field with Valid Clinical Licensure

• Experience in working with Microsoft Office programs and data base management essential
• Must be able to read, write, and speak English.
