Pathways Hospice has an immediate opening for a Bereavement Manager. LCSW preferred.

Pathways for Grief & Loss is a nationally acclaimed bereavement counseling program. We offer individual, couples and family grief counseling for clients of multiple age groups and experiencing all types of loss. To support the needs unique to losing a parent, spouse, child, friend or coworker, Pathways offers specific services and programs themed on these relationships. Our team of Licensed Clinical Social Workers and Licensed Professional Counselors also facilitate support groups and provide bereavement education and outreach to area schools and businesses. Location: Ft. Collins & Greeley

Status: Full-time

Schedule: Monday-Friday, 40 hours

Pay Range: $68,702 - $82,638

Supplemental Pay: Based on position, schedule and/or availability: Overtime

Mileage and Expense Reimbursement: Maximum Allowable Federal Mileage Reimbursement Rate

REWARDING WORK YOU WILL DO:

Manages programs and staff, providing grief counseling and support for bereaved children, adults and families, as well as persons with a serious illness. Manages the continuity of care between the direct-care and the bereavement components of hospice services. Facilitates bereavement support groups and community outreach programs.

  • Supervises bereavement services for hospice families. Ensures bereavement assessments and plans of care are completed in a timely manner following the death of a patient and in accordance with Medicare Conditions of Participation (COPs).
  • Monitors bereavement programs and events, including community counseling and support groups. Delivers education for community, volunteers and staff.
  • Monitors policies and procedures to assure compliance, quality of care and consistency with hospice mission and values.
  • Ensures maintenance of all documentation in accordance with HIPAA practices and DORA regulations.
  • Supervises bereavement counseling staff and contracted group facilitators. Coordinates and oversees orientation and training of new staff and interns; assists in determining appropriate staffing levels.
  • Maintains professional relationships within the community; participates in the development of new bereavement programs and services to meet community needs.
  • Provides bereavement support and grief counselling to hospice families; facilitates grief support groups when needed.
  • Participates in quality assessment and performance improvement (QAPI) programs at a departmental or agency-wide level.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Minimum Education: Master’s Degree in Social Work, Counseling, Psychology or related discipline. LCSW preferred.
  • Minimum Experience: One to three years of experience as a counselor or social worker in a medical or mental-health setting (preferably home care or hospice); one year management experience.
  • Required License: Colorado license in Professional Counseling or Clinical Social Work. Valid Colorado driver’s license and auto insurance.

PHYSICAL REQUIREMENTS:

  • Ability to lift/carry a minimum of 30 lbs

Care Synergy provides mission support services to better equip Pathways Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
